Black Canyon Middle School

Students who are participating in Boys Basketball will need to fill out the Middle School Extracurricular Participation Form and submit a physical and a signed parent consent form.

Parents/guardians are now able to upload a doctor’s physical digitally & digitally sign the consent forms. If Black Canyon MS does not have a physical on file or a consent form, students will not be allowed to participate without proper documentation.

Pay to play for basketball is $50.00 and all students will need to have an activity card, if they do not they can be purchased from the School Bookkeeper for $15.00.
