All participants must have a current physical and completed the registration form.
Students who are participating in Cross Country will need to fill out the Middle School Extracurricular Participation Form and submit a physical and a signed parent consent form.
Middle School Extracurricular Participation Form
Parents/guardians are now able to upload a doctor’s physical digitally & digitally sign the consent forms. If Black Canyon MS does not have a physical on file or a consent form, students will not be allowed to parcipated without proper documentation.
