Open Gyms are open to incoming 7th graders and current students at Black Canyon MS.

         Students who are participating in Girls Volleyball will need to fill out the Middle School Extracurricular Participation Form and submit a physical and a signed parent consent form.

Parent Permission and Physical Form

Parents/guardians are now able to upload a doctor’s physical digitally & digitally sign the consent forms. If Black Canyon MS does not have a physical on file or a consent form, students will not be allowed to participate without proper documentation.

Pay to play for is $50.00 and all students will need to have an activity card, if they do not they can be purchased from the School Bookkeeper for $15.00.
